The management software/database you created has been highly successful, so you have decided to license it to other businesses. This is a big change to your business mission, but you are confident in your decision.

Since you are busy running your business, you must hand over the design, expansion, and maintenance of your database software to your new assistant.

Instructions:

For this assignment, you must create a cohesive guide that provides your assistant with enough information to take over and manage your database. Mary is a 35 years old electrical engineer who presents to the office for evaluation of a rash on her face that has been present for 1 week. She denies new soaps, detergents, lotions, environmental exposures, medications, and foods. The rash is across her face and the bridge of her nose. She states that she first noticed it after spending a week hiking and camping in the Appalachians. The lesions itch and are painful. She has not tried anything to make it better, but she has noticed that going outdoors makes it worse. She denies any spread of the rash to other areas. She has never had this rash before.

She has noticed some increased fatigue, fever, and weight loss. She denies headache, sore throat, ear pain, nasal or sinus congestion, chest pain, shortness of breath, cough, abdominal pain, and pain with urination, constipation, or diarrhea. She does have mouth soreness. She has noticed some increased muscle aches and pains, which are worse in the hand and wrist. She denies early morning joint stiffness or difficulty with being able to move in the morning. She denies temperature intolerance, polyuria, polydipsia, or polyphagia.

She had a tonsillectomy at age 9 for chronic strep throat infections. She has been healthy as an adult. She has never had children. She has never been hospitalized for any reason.

Her family history is significant for a mother with rheumatoid arthritis. Her father is healthy.

She does not smoke; she drinks a glass of wine nearly every night with her dinner; she denies illicit drug use. She completed a master’s degree in engineering. She has lived with her boyfriend for the past 5 years.

Patient is an alert young woman, sitting comfortably on the examination table. BP 112/66 mm Hg; HR 62 BPM and regular; respiratory rate 12 breaths/min; temperature 100.3°F. Several erythematous plaques scattered over the cheeks and the bridge of nose, sparing the nasolabial folds. Normocephalic, atraumatic. Sclera white, conjunctivae clear; pupils constrict from 4 mm to 2 mm and equal, round, and reactive to light and accommodation. Oropharynx moist with erythema in the posterior pharyngeal wall; no exudates; shallow ulcers in the buccal mucosa bilaterally. Neck supple without cervical lymphadenopathy or thyromegaly. Full range of motion; no swelling or deformity; muscles with normal bulk and tone.

Fly-By-Night University (FBNU) has several campuses and has recently decided to centralize their student records. The university’s main office needs to develop a new system to house student records and course information. You have been hired to make recommendations regarding the design of the new system. You have met with key personnel for the purposes of requirements gathering. You have analyzed the current system and the goal for the future system.

These are the items you have determined:

1. Faculty are grouped based on their subject matter expertise. For example, there is English faculty, CIS/IT faculty, Management faculty, etc. Every faculty group has a dean. Each professor is assigned a specific school and campus. Faculty are allowed to teach any course within their subject matter area.
2. Courses will have a title, but are categorized by a course code. Some courses have prerequisites, which need to be recorded so that students are not able to bypass the prerequisites.
3. There are various campuses within the state. For example, there is a San Francisco campus and a Los Angeles campus.
4. Each campus may have its own schools. For example, the Los Angeles campus has a School of Business, a School of Liberal Arts, and a School of Technology.
5. Each school offers multiple programs of study. For example, the School of Business may offer degrees in management, marketing, accounting, etc.
6. Students are required to be enrolled in a specific program of study. Each program of study has a set of core courses that need to be completed in addition to elective courses.
7. The university identifies each student by name, date of birth, student ID, and program of study.

Using the results from requirements gathering, write a report in which you:

1. Analyze the requirements and write a proposal for the university’s new database and system. You will need to organize all of the required data elements in a manner understandable to management. Your proposal should include the following:
a. Provide an entity-relationship model (ERM) that will fully describe the new data structure. Be sure to include all data elements.
b. Describe the assumptions or limitations for each relationship. For example, students are only allowed to be enrolled in one degree program at a time; faculty are confined to teaching courses within their subject matter area.
2. In your ER diagram, depict the primary key and foreign keys for each entity.
3. Suggest at least three Web applications that need to be developed for the new data management strategy.
4. Depending on the required time and resources for a project of this magnitude, the university has indicated they are open to outsourcing the project. Perform some research and look for vendors who develop registrar and school management database systems.
5. Suggest three vendors that could develop and implement a school-management database system. Explain the benefits of choosing from one of these vendors.
6. Compare and contrast the key benefits for each system. Examples might be full customization, a cloud-based model, scalability, strong support.

Avoid writing in an “essay” style. You are not writing an academic research paper; you are writing a proposal for a simulated real-world scenario.

What is a Stress Test?

There are a couple of definitions for stress test, including actual tests that people can take to score their emotional stress levels. More commonly in medicine, the stress test refers to measurement of cardiac function, when the heart is working hard or under “stress.” This shouldn’t be taken as negative stress to the heart, but simply means a demand on the heart to work harder either through exercise, or through injection of a medication.
A number of reasons exist why doctors, and often cardiologists, would order a stress test. These could be to rule out or rule in conditions like greater risk of heart attack, abnormal heart rhythm patterns, or presence of diseased coronary arteries. Sometimes patients present with symptoms like difficult breathing or heart pain that aren’t easily explained and the test could be used to diagnose a cause or to rule out the heart as the cause of symptoms.
Several different variations of the stress test exist. In the exercise electrocardiogram (EKG) test, people will use a treadmill or bicycle to increase heart rate while the EKG is carefully watched for any irregular signs. The test usually begins with a resting EKG reading, and then a person will continue to wear EKG monitoring stickers as they begin to exercise.

Typically the exercise portion of this test isn’t that long, though it may feel long if people don’t exercise regularly. Generally most people elevate their heart rate up to the desired area in about 10 minutes, and EKG readings will be taken as this occurs. Occasionally this test finishes with an echocardiogram or sonogram of the heart, but this is not always the case.
An echocardiogram stress test is slightly different and may be performed in several ways. This looks at pictures of the heart when it is working, and usually begins with administration of medications like dobutamine or adenosine. These medications replace exercise and cause the heart to beat very fast, as if exercise were occurring, which can be an uncomfortable, though fortunately short-lasting sensation. Both EKG and echocardiogram may be performed, and as with the exercise test, vitals like blood pressure are monitored carefully.
Since there are so many possible variations of the stress test, it is difficult to instruct people on what to expect or how they should prepare. The best instruction usually comes from the cardiologist performing the test. In general it might be said that if the test involves physical activity, people should wear comfortable clothes and shoes that are suited to a workout. If an echo is involved, women will be asked to remove bras, so wearing one that is easy to remove makes good sense. Most tests do ask people not to eat for 12 hours prior to testing, and it may be wise to schedule these tests for early morning to avoid daytime fasting.
Another area that cardiologists should fully explain to patients is that they can and should voice discomfort. If testing suddenly involves acute pain, even if not heart related, patients can stop. Most will have the option of doing other forms of stress tests if exercise is uncomfortable or too difficult to do.
The last thing that people may want to know is if an order for a stress test always means a diagnosis of cardiac problems. It doesn’t. Lots of people have these tests and are shown to be in very good shape. However, should the test reveal cardiac issues, people may take comfort in the fact that catching heart problems early is far more favorable when it comes to treatment outcome.
