Case Study #3: Insomnia and Sleep Disorders 

S.H., age 47, reports difficulty falling asleep and staying asleep. These problems have been ongoing for many years, but she has never mentioned them to her health care provider. She has generally “lived with it” and selftreated the problem with OTC Tylenol PM. Currently, she is also experiencing perimenopausal symptoms of night sweats and mood swings. Current medical problems include hypertension controlled with medications. Past medical history includes childhood illnesses of measles, chickenpox, and mumps. Family history is positive for diabetes on the maternal side and hypertension on the paternal side. Her only medication is an angiotensinconverting enzyme inhibitor and diuretic combination for hypertension control. She generally does not like taking medication and does not take any other OTC products.

Diagnosis: InsomnIa

1. List specific goals of therapy for S.H.

2. What drug therapy would you prescribe? Why?

3. What are the parameters for monitoring the success of the therapy?

4. Discuss specific patient education based on the prescribed therapy

5. List one or two adverse reactions for the selected agent that would cause you to change therapy.

6. What would be the choice for second-line therapy?

7. What OTC and/or alternative medicines might be appropriate for this patient?

8. What dietary and lifestyle changes might you recommend?

9. Describe one or two drug–drug or drug–food interactions for the selected agent.

To organize and strategize data collection.

One way to help organize data is by coding. Coding refers to the identification of topics, issues, similarities, and differences that are revealed through the participants’ narratives and interpreted by the researcher. This process enables the researcher to begin to understand the world from each participant’s perspective(Sutton & Austin, 2015). Coding can be done by hand or by a software program to review the gathered transcripts of data and categorize specific topics in the research. It also can help establishing the credibility of research. Having an additional researcher code the same transcript and then to discuss any similarities and differences in the 2 resulting sets of codes. This simple act can result in revisions to the codes and can help to clarify and confirm the research findings(Sutton & Austin, 2015).

Entity Relationships.

-If your design is not in at least third normal form, revise it so it is in at least third normal form. Provide the revised design in addition to the original. Justify why the new design is now in at least third normal form with evidence from the design and the definitions of the normal forms.

-Provide five specific examples of data anomalies (including at least one insert, update, and delete anomaly each) that might happen if your database were not in at least third normal form. Provide sample non-normalized data and tables, the action taken, and the resulting anomalous data. Explain why your design avoids each possible problem.

Label each of the screenshots in the document to show which part of the assignment each screenshot is supposed to fulfill.
