Community Assessment

The purpose of this assignment is to determine the healthcare needs in a particular area. Follow these guidelines and present the information according to the grading rubric.

Assessment Focus

A. Select a census tract representing a neighborhood or community located in your area. Gather a broad range of assessment data pertaining to this location.

Community Description

This information would come from a windshield survey (pg. 192) and through interviews. You can use the “Learning about the community by foot (or car)” document to help guide your windshield survey, but this is just a guide. Your windshield survey should be tailored to the needs of the specific community you choose.

Describe relevant features affecting population’s health.

Housing of population in location and industry.

Public resources: sanitation, protection, transportation, and parks/recreation.

Schools

Significant environmental factors or hazards

Means of communication available to client population

Local community leaders and projects

Population Description

A. Demographic characteristics (sex, age, race, ethnicity, income, employment, and education levels)

B. Comparisons over time (use 2000 and 2010 Census Data) and to larger relevant populations, such as the city you live in.

1. Census Data is available on line from the CDC and Department of Vital       Statistics.

Population Health

Vital statistics (two time periods and comparative location – just because another town is close does not make it a comparative location, a comparative location is similar in size and make-up of the community)

Leading relevant health indicators including numbers and rates (if available)

Other identified health problems

Analyze findings and discuss implications drawing from text and relevant articles.

Health Resources (this may come from your windshield survey, community interviews, or various internet resources)

Health resources available to local population

Health resources lacking

Local public health resources and initiatives.

Health Problems

List and discuss the major and minor health problems (differentiate between the major and minor problems)

Prioritize the top two

Program Objectives
