WVIPA charges membership dues (via a membership agreement) to its member physician groups; the dues pay for WVIPA’s operating expenses and organizational costs. In return, WVIPA credentials providers, negotiates insurance contracts and reimbursements for its members, and performs some basic data analytics functions. WVIPA’s membership is distributed disproportionately among six counties. Two thirds of WVIPA’s membership (and nearly all of its highly coveted physician specialty groups) comes from two counties: McKinley and Grant. McKinley and Grant counties are home to prosperous, growing communities that attract both young physicians and those physicians seeking to retire. Neither county is designated as a health professional shortage area. Practices in McKinley and Grant counties receive substantial reimbursements on a fee-for-service basis because they provide high-quality specialty care. These practices have invested in electronic medical record (EMR) systems and data analytics functions. WVIPA’s remaining membership comes from four (predominantly rural) counties: Taylor, Harrison, Polk, and Monroe. All four counties are designated as health professional shortage areas. Most of the practices in Taylor and Harrison counties are solo practitioner offices; these physicians rarely use EMR systems. WVIPA reports that the quality of care offered by these physicians is excellent and readmission rates are low, but that gathering data from these practices can be time and labor intensive without EMR systems. These practices report that they do not make enough money to invest in EMR systems. Physicians in Polk and Monroe counties typically use EMR systems, but access in those counties is poor. Patients must wait more than three months for a preventive care visit. These practices typically do not engage in care coordination activities because access is poor, and patients who need follow-up would not be able to get in to see a provider immediately. WVIPA’s most valuable function (according to its member physician groups) is its role in negotiating and securing favorable reimbursement rates from insurance companies. When WVIPA negotiates with a payer, it leverages its physician network—which is a monopoly, in some areas—to increase reimbursements. WVIPA has negotiated Medicaid, Medicare, and commercial contracts for its physician members. All physician groups that are members of WVIPA must agree to participate in (and accept payment from) the insurance contracts that WVIPA negotiates and signs. WVIPA uses a contract committee (the “Committee”) with representation from 12 major member physician groups to secure approval for its negotiated contracts. If the Committee approves the contract, WVIPA can sign it. If the Committee does not approve the contract, WVIPA must refuse to sign the contract. Over the past two months, WVIPA has been negotiating with the largest insurer in the region. WVIPA’s existing contract with the insurer will terminate by expiration in one month.
