This assignment is designed to support the work for your health promotion project.
1. Write a mission statement
2. Complete the objectives worksheet (located in the assignments folder in SacCT)
3. Write in narrative format the objectives
You should list at least three supporting program objectives (one for each level individual family/community and policy level) for your proposed community health promotion and policy program.
Use Chapter Six of the HPP text to assist you:
1. Mission Statement Complete the worksheet:
Specific measurable achievable realistic and time-phased (SMART) Program Objectives
Include at least one objective for each level
One individual/ family level objective
One community/ agency level objective
One policy/ legislative level objective
2. Then write out each objective in the format below noting in ( ) the required part of the objective
Each objective must include the following elements:
Outcome (what)
Priority Population (who)
Conditions (when)
Criterion (how much)
Example: Individual Level: By January 1 2009 (when) ninety-five percent (how much) of the CSUS Nursing students in N178 (who) will demonstrate their ability write a well-written health promotion
objective (what).
Community or agency level: By January 1 2011 (when) 70% (how much) of the Hmong community (who) will demonstrate knowledge of where to find services for persons with developmental disabilities
(what).
Policy or legislative level: By July 15 2020 (when) the California legislature (who) will enact new legislation that requires that all children residing in California (how much) be screened for
autism by 18 months of age using culturally and linguistically appropriate screening tools (what).
3. Submit your objectives worksheet with your objectives written in SMART format with the words in ( ) When How Much Who What. See the examples above
Format and Submission Instructions:
1. Submit the Mission Statement Objective Worksheet and the written objectives in one document to SacCT.
2. Include a header on each page.
3. Use Times New Roman 12 pt font. The worksheet should not be double spaced however the written mission statement and objectives should be double spaced.
4. No APA references are required for this assignment.
5. Submit via SacCT assignment drop box.
