Business Requirements Template and Samples for Software Design

I need a Business Requirements template with examples formatted in an Excel Spreadsheet.  The requirements example can be for development for web based software application.  Any type of software application does not matter, just need to see format and examples of how to write requirements.  I business requirements, functional and non-functional requirements.  Assignment attached need the assignment completed by tonight if possible.
