Chapter 5 – Ambulatory Care

Assigned Readings:  Chapter 5

Answer the following questions: Identify the major factors that have resulted in the shift in utilization from inpatient hospitalization to ambulatory care services. Describe the implications of this shift for hospitals, consumers, and the health care delivery system as a whole. In what ways have private practice physicians responded to the expansion of hospital, health plans’ and other organization-sponsored ambulatory services?  What are the implications of those responses for the patient, for the consumer, and for the health care industry as a whole? Hospital emergency departments continue to be used as an ever more important source of primary medical care by large numbers of the community’s medically underserved population. What are the implications of this practice for the patients, and on health care costs and quality of care? What would you propose as a means to change this situation?  Almost two-thirds of all surgical procedures are now performed in ambulatory surgery facilities. Discuss the reasons for this shift of surgery from the inpatient setting and its effect on hospitals and consumers. Some people support the role of public health departments in providing preventive and treatment services as necessary to fill gaps in the system for the medically needy. Others believe these services are more efficiently and effectively provided through private organizations. What is your position on this issue and why?

Dollars and Dentists

Now that you have read, reflected and analyzed various ethical and moral issues in health I’ve selected a case study that I want you to respond to.

The first is a Frontline edition of a program titled Dollars and Dentists aired on June 26, 2012.  

The program was a documentary developed by the Center for Public Integrity and hosted by the Public Broadcasting System (PBS).  The program presents an overview about the poor condition of oral health in this country and some of the issues associated with a broken health care system.  Specifically the program discusses the high cost of dental care, the lack of insurance to cover oral health issues, and how these costs leave many people unable to afford dental care services.  It also calls into question the way corporate America has responded  to fill this need.   Several business models have emerged with big corporate backing. 

While viewing the video you should be able to recognize a number of ethical and moral issues that arise related to access to health care, quality of care, and financial exploitation of disparate populations.

The healthcare/medical community has its own code of ethics, which is outlined in the American Medical Association’s Code of Medical Ethics. Then, by analyzing the issues through these steps: Analyze the specific requirements of a code of ethics. Identify three critical elements of the AMA’s code that you believe are most important. Examine the differences and similarities between the AMA’s Code of Medical Ethics requirements and the code requirements of the three hospitals that you researched. Explain why certain individuals and/or groups (please name these) should follow a hospital’s code of ethics.
