PICOT Question:
In hemodialysis patients (P), would timely intervention by the Nephrology Nurse Practitioner such as intentional patient education on increased risk of blood stream infection with usage of a CVC, improved infection control practices, and timely referral for placement of a permanent access such as an AV Fistula or AV Graft (I), compared to no intervention (C), decrease mortality rates related to CLABSI (O) within the first month of hemodialysis treatment (T).
Research conducted by (See, Shugart, Lamb, Kallen, Patel, & Sinkowitz-Cochran, 2014) found that patients on hemodialysis are at a much higher risk for bloodstream infections directly related to repeated access to the bloodstream via central venous catheter for dialysis treatment. Staphylococcus aureus is the most noted bacteria found and accounts for 86% of these BSI’s. This study advises that risk of BSI in these patients are 100-fold greater than that of the general population, and that providing education to all patients to include access care, hygiene, signs and symptoms of infection, care of CVC at home, and risks associated with catheter use is highly suggested. Lastly, (See, Shugart, Lamb, Kallen, Patel, & Sinkowitz-Cochran, 2014), found that patients believe that education on infection control and risk for BSI should begin as early as possible in the process of dialysis with regular reinforcement. According to (Suzuki, Satoh, Nakamura, Horita, Seki, & Moriya, 2016), a study revealed that CVC use poses a much higher use for infection than a fistula or graft. This same study shows that infection related hospital admissions in dialysis patients are higher for CVC’s at a 50% higher risk of septicemia versus for those that use a fistula or graft. Lastly, study also notes that improvements in infection control measures, education, and hygiene reduces occurrence of bacteremia.

Picot Question: In elderly patients admitted to the medical-surgical unit, how effective is the implementation of hourly rounds, use of bed alarms and huddle notes, when compared to the use of previous use of bed alarms only, decrease the incidence of falls during the length of hospital admission stay.
Falls are common events for hospitalized older adults, resulting in negative outcomes both for patients and hospitals. The Center for Medicare and Medicaid (CMS) has placed pressure on hospital administrators by identifying falls as a “never event”, resulting in a zero falls goal for many hospitals. Staff nurses are responsible for providing direct care to patients and for meeting the hospital no falls goal (King, Pecanac, Krupp, Liebzeit & Mahoney, 2018). Preventing patient falls is one of the biggest challenges facing health care workers today. Quality improvement (QI) initiative aimed to promote patient safety by improving adherence to an existing hospital-approved fall prevention protocol. Specific aims of the initiative were to evaluate the impact of using a fall prevention checklist by implementing fall prevention protocol and the incidence of falls on participating units (Johnston & Magnan, 2019). Commonly used fall prevention interventions include fall risk assessments, fall risk alerts, environmental modification, use of assistive equipment, staff and patient safety education, medication management, staff assistance with transfers and toileting, hourly rounding, and bedside change-of-shift reports. Prevention interventions are presumed to be helpful but need to be used consistently (Moraes, Almeida, Dornelles, Reis, Godinho & Junior, 2016).
