A clinical documentation program needs to be established at Scholastica University Hospitals.  From the findings and information from the last documentation audit, it was clear that the documentation at Scholastica University lacked the key elements needed for high-quality clinical documentation.  You were just hired as the Health Information Director.  Your first assigned project is to create and establish a system wide clinical documentation program to help improve the quality of the documentation within the patient record.  

Deliverable: Write a 2 page report (paper) on your plan to create and implement a system wide clinical documentation program at your organization.  Consider your report the formal documentation your are bringing to the Chief Financial Officer, Chief Medical Officer, and the Chief Executive Officer on why a CDIP must be developed.
