In 2010 internal confidential inquiries were made public disclosing “instances in which management flouted safety by neglecting aging equipment, pressured or harassed employees not to report problems, and cut short or delayed inspections in order to reduce production costs.” A history of inquiry 2001 (Alaska) – Key equipment needed for emergency shutdown was neglected, including safety shutoff valves and gas and fire detectors similar to those that could have helped prevent the fire and explosion on the Deepwater Horizon rig in the Gulf. 2002 (California) – BP falsified inspections of fuel tanks at a Los Angeles-area refinery and more than 80 percent of the facilities didn’t meet requirements to maintain storage tanks without leaks or damage. 2004 (Alaska, California, Texas) – An inquiry found a pattern of intimidating workers who raised safety or environmental concerns. Managers shaved maintenance costs by running equipment to failure. Accidents resulted, including the 200,000-gallon Prudhoe Bay pipeline spill in 2006, the largest ever spill on Alaska’s North Slope. 2005 (Texas) – An emergency warning system failed before a Texas City refinery exploded in a ball of fire. Following the fatal accident, Tony Odone (BP spokesman) stated the company adopted a six-point plan to update its safety systems worldwide. 2009 (Texas) – The Occupational Safety and Health Administration fines BP $87 million for failing to make safety upgrades.
