SCENARIO
Susan is a patient on a rehabilitation unit. She has a diagnosis of schizoaffective disorder and borderline personality disorder. She often experiences mood swings and constant feelings of paranoia and being watched. She copes with these feelings by occasional self-harm and can sometimes be aggressive and disturbed, when bothered by paranoid feelings and a raised mood.
The past 72 hours have been particularly disturbed for Susan. She has expressed concerns that both staff and patients are plotting to poison her food, and she has made indeterminate threats to “retaliate” if she feels she has been poisoned.
You receive a phone call from Susan’s sister, who lives in a different city. Susan is visiting with her 12 year old nephew and would like to visit that evening. Susan is very fond of her nephew and often talks of him and spends her limited money on buying him gifts.
Staffing levels are adequate but low on the ward that evening and several other patients are quite unwell and agitated. Outline a management plan for this situation.
